CLAIM PROCEDURES

How to File a Personal Accident Insurance Claim

In the event of any happening which may give rise to a claim under this Policy, You must:

Written Notice

I. Advise us in writing with full particulars as soon as possible but in any event not later than 3 weeks of the occurrence of such incident

II. Fill in a Personal Accident Insurance claim form 

Provide Documents

  1. Submit to us an accident investigation report, if any
     
  2. Forward at your expense the death certificate of the Insured Person and the coroner's report, if required by us
     
  3. Provide the necessary assistance to us to enable us to arrange for a post mortem examination at our expenses
     
  4. Forward at your or the Insured Person's expense all certificates and information required by us
     
  5. Submit himself or herself for all the medical examinations from time to time as required and arranged by us at our expenses
     
  6. Submit original medical evidence and/or medical receipts.
